In order to accurately serve your needs and get started on your new patient file, please fill out the following New Patient Form.
This includes you (or your child’s) basic information, medical history, and specific goals for your dental needs. Please make sure to complete every page to the best of your knowledge. Please add any details you think are important to your medical history, including facial injuries, chronic dental problems, or any diagnosis related to sleep apnea or other breathing issues. In addition, any relevant family history should also be included.
If there is information that you do not know or not sure of, you can leave that blank and we will work to fill it in at a later time. At Vickie Greenberg Orthodontics, we are dedicated to providing the highest level of expertise possible for all your orthodontic needs. Board-certified Dr. Greenberg specializes Invisalign®, Invisalign Teen®, maxillofacial orthopedics and non-surgical and non-extraction orthodontic treatment.
